Qualification Criteria

To qualify as an acceptable assessment or treatment program or provider, a program or provider must meet all of the following criteria:

  1. A program must be licensed to operate as a treatment program by the state in which it is located, and a provider must be licensed to practice as a health care professional or certified social worker by the state in which the individual practices.
  2. The program staff or provider must have at least three years of experience treating the specific impairment for which the referred participants will be treated.
  3. The program or provider must designate an appropriate health care professional who will be responsible for responding promptly to requests by the HPRP for evaluations of participants and requests for information about participants.
  4. The program or provider must agree to submit required and requested reports on each participant to the HPRP in a timely manner.
  5. The program or provider must treat chemical dependency as a primary disease, not a symptom of some other condition.
  6. Assessment and treatment shall be provided by a multi-disciplinary team.
  7. The treatment regimen shall follow a 12-step approach.
  8. The treatment regimen shall be structured and address the physical, mental, emotional and spiritual needs of the participant.
  9. The assessment and treatment staff shall be experienced in assessing and treating the chemically dependent health professional.
  10. The treatment staff shall conduct a comprehensive assessment and evaluation of patients upon admission and design a treatment plan around each participant's needs.
  11. The treatment regimen shall facilitate and encourage participant's involvement in a family treatment component.
  12. The treatment regimen shall provide for participation in A.A. and/or N.A. recovery groups for health professionals and other appropriate self-help groups while the participant is in primary treatment.
  13. An individualized continuing care plan shall be developed for the health professionals before discharge from primary treatment, including:
    · continuing care group
    · individual, group, or family therapy, as needed
    · self-help groups, including A.A. and/or N.A.,
    · health professional support group
    · obtaining a self-help group sponsor
    · obtaining a health professional sponsor
    · employment plan
