How do you report to HPRP?
In order to report a suspected impaired health professional, contact HPRP’s Intake Department at 800-453-3784. The following information is required in order to make a referral:
- Your name and a contact telephone number
- The suspected impaired health professional’s name, profession and mailing address
- First-hand information which leads you to believe the health professional is impaired .
